Office Visits

Please arrive 15 minutes before your scheduled appointment so you have ample time to complete any paperwork.

You can print patient forms from the web site and bring them in completed, although we ask that you still arrive 15 minutes early to complete any psychological scales that the Dr. Guerrero may want you to complete.

If you schedule a New Patient Appointment and find that you are unable to come in, please call the office AT LEAST 48 BUSINESS HOURS IN ADVANCE so that Dr. Guerrero can use his time most effectively. Otherwise, you will be charged the full amount of $375 for the missed appointment time. As a courtesy, we will call you one or two days before your scheduled appointment to remind you of the appointment, but it is your responsibility to remember the appointment.

If you schedule a Follow-Up Appointment and cannot keep your scheduled appointment, we require that you call the office to cancel or reschedule that appointment AT LEAST TWENTY-FOUR (24) HOURS IN ADVANCE. Failure to follow this policy will result in having to pay the full amount of $187 for the missed appointment. As a courtesy, we will call you one or two days before your scheduled appointment to remind you of the appointment, but it is your responsibility to remember the appointment.

Your appointment time is especially reserved for you. Dr. Guerrero does not double book appointments, but prides himself on giving each patient his full attention and time for the entire appointment. If you do not cancel your appointment or do not show up for it, Dr. Guerrero is unable to see another patient because the time slot was allotted specifically for you. "No shows" and missed appointments will be billed.

Dr. Guerrero does not accept insurance, however we do provide you with an insurance-ready receipt that gives the patient Dr. Guerreros Tax ID Number, the date of service, the proper diagnostic and procedure codes upon each visit. It is ready to be filed with your insurance company for reimbursements according to each patient's health plan.

Accepted Forms of Payment

Cash, Check, MasterCard, Visa, and Debit-Cards. Please note that any non-sufficient checks will be charged a $25.00 NSF fee in addition to bank fees.

The Psychiatric Evaluation and Treatment Plan:

Psychiatric care begins with a 60-minute Extended Comprehensive Psychiatric Exam. Dr. Guerrero will take a careful history of the problems you or family member are experiencing, and will also take a family history. Dr. Guerrero uses psychiatric and psychological scales to help gather information, not only about signs and symptoms, and problems you and your family are facing, but to also substantiate his diagnosis. These scales are helpful in determining and showing a patient's progress too. From this understanding, Dr. Guerrero will create a highly personalized, multifaceted treatment plan for the patient. Treatment may include medications, therapy or counseling.

Medication Management or Follow-up Appointments:

Follow-up appointments are 30 minutes. Dr. Guerrero consistently prescribes the lowest dose of medication possible to minimize side effects. The goal is no side effects at all. Dr. Guerrero respects your budget and will prescribe low cost drugs if needed. Wherever and whenever possible, we will try to provide samples to patients.

Privacy Practices

Our Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. It is given to all new patients, and a copy is available upon request.
